Effective September 1, 2014, Pastor Medical Group requires that all fees be paid when due. Co-payments are due at the time of service. Co-insurance and deductibles are due immediately following the processing and payment by your insurance(s). You must have a credit card on file authorized to pay your portion of the bill. If you are unable to provide a credit card, please speak to Dr. Pastor to make other arrangements. If you are self-pay, the entire amount is due at the time of service.  

Emergencies and Urgent Needs

If you have a life threatening emergency, go immediately to the hospital emergency department or call 911 to summon the life squad. Ask the hospital to contact Dr. Pastor. If you have an urgent non-life-threatening problem, please call the office right away. During regular office hours PMG staff will arrange for an appointment. If you call after hours leave a detailed message and the doctor on call will be paged. Please keep your telephone line clear so that your call can be returned.

Medical Records

​During your first visit to the office, a permanent record for you will be established. Your record will be kept in strict confidence and will be released only with your permission. If you wish to transfer your record, you need to sign a release form and a charge will apply.

Prescriptions and Refills

​If you would like to request a new medication, please call the office for an appointment. If you need a prescription refilled before your next office visit, have your pharmacist call the office during regular hours. You may also call and request a refill.

Medical Test Results

​Dr. Pastor will review your test results. You will be informed of your results either by phone, e-mail, regular mail or patient portal. If additional tests or treatments are required, PMG staff will make the necessary arrangements and  request that you make a follow-up appointment to discuss the findings with the doctor.

Insurance and Fees

PMG participates in most insurances, but make sure that Dr. Pastor or PMG is included in your provider panel. Please remember that insurance coverage is a contract between you and your insurance company, and you are responsible for payment when the account is due. PMG is happy to submit all claims to your insurance. Co-payments and deductibles are expected at the time of your visit. Medicare is accepted.


​Many HMO insurance plans require a referral from your primary care physician before you see a specialist. PMG Staff will be happy to assist you in this process.

Prior Authorizations​

Your insurance may require a prior authorization prior to a test or for a medication. PMG staff will be happy to assist you.  Please provide us will all the necessary information in order to complete your request in a timely manner.

No Show Policy ​

If you are not able to keep your appointment, please call the office. Leaving a voicemail to cancel before 8:00AM on the day of your appointment will not incur a fee. Failure to cancel without notifying us will result in a $50.00 charge.

Disability forms and reports, FMLA forms and letters​

​Letters and forms that need to be completed by your doctor require time. Please give the office at least one week in advance before the form or letter is due. There will be a charge for all reports and copies. Pre-payment is required. Contact the office for fees.
