Dear Patient,

It has been an honor and a pleasure taking care of all your healthcare needs over the years. I have been practicing medicine for 30 years and, as many of you know, over the last 3½ years I have been pursuing a Master of Public Health from the George Washington University and it is with great pleasure that I announce my graduation this summer.

Obtaining this degree opens the door for me to work in global health and improve the health of large group of people. It has been a dream of mine to help the less privileged in their pursuit of health as defined by the World Health Organizations as “…a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity.” This means I am taking a sabbatical from Pastor Medical Group to pursue research in public health. I start this adventure in June going to Nicaragua to serve under AMOS Health and Hope.

I understand that it may be difficult for you to understand my decision, however, I hope you can support me in pursuing my next endeavor. Over the next two months my staff and I will be available to assist you in your medical needs while you find a new doctor. Throughout June, Havier and Sergio will make a CD containing the last six years of your medical records. You will be notified when you can pick up the CD or you can request that it be mailed. You can share this information with your new doctor. However, I suggest you keep the original CD as it will be the only free copy of your medical records. I will be seeing patients and providing prescription refills until May 25th, 2018.

A: Dr. Pastor does not recommend any particular doctor or facility.

Q: What other doctors are in the area?
A: There are many doctors in the area. You can call the numbers below to get a referral to a doctor near you. You can also call your insurance company to help you find a new doctor.

Q: What about my medical records?
A: You will be provided one (1) free copy in a CD format of your medical records in June. If you need a copy sooner, please contact us. Additional copies will incur a fee based on the Ohio Revised Code 3701.741.

Q: If I have over paid, when will I get a refund?
A: Any overpayments will be processed during June and refunded in July.

Q: What about refills?
A: You can request refills on medications until May 25, 2018. We will not do refills after this date.

Q: What about automatic refills?
A: You need to call your pharmacy to stop automatic refills. Medications will not be refilled after May 25, 2018. Please call your pharmacy giving them information about your new doctor.

Q: How do I contact the office after May 25, 2018?
A: You can call 513-939-0333 and leave a message for non-medical questions. For all your other medical needs, you will need to call your new doctor.

Q: What about appointments scheduled after May 25, 2018?
A: All appointments with Dr. Pastor and the lab after May 25, 2018 will be cancelled.
